Four pages including note page with abbreviations and numbers, 1955 8" x 10" ruled paper page of calculations and text, both in his own hand. Also includes preprinted 1956 request for military leave signed by Slayton and folded oversized Flight Data Sheet entirely written in Deke Slayton's hand. Ken Havekotte COA included.
