Karloff, Boris (1887-1969) Actor best known for "Frankenstein." Document Signed twice, once on the front and again on the back, 3¼" x 7¼", Los Angeles, Ca., 17 May, 1966. Karloff's tax-withholding form for his Personnel Record carries his birth name, William Henry Pratt, as well, but he signed as Boris Karloff both times, in blue ink. On the back, the first three letters of "Karloff" are affected by a punch hole; the signature on the front is nice and bold.
